New Patient Paperwork

• Are your clients referred to you? Most patients are referred to our center from other patients as well as health care practitioners (i.e. chiropractors, medical doctors, nurse practitioners, optometrists, physical/occupational therapists, acupuncturists, nutritionists, massage therapists, etc.)
• Do you work with insurance? We work with many insurance companies. An evaluation of the coverage will be determined and it may be used to help reduce some of the cost of care.
• Do you accept credit cards? Yes! We accept Visa, MasterCard, and Discover. You may also pay by cash or check.
• Do you take payment plans? Yes! We offer several payment options with a 12-month interest free program being the most popular plan. Options are discussed when formulating a care plan.
